If your product is in need of service, then read the directions carefully and download the service request form below. This warranty is applicable only if the Lumens product was purchased new, originating from Lumens Integration, Inc. (USA).

For products not purchased from Lumens USA are not covered under our warranty policy. For support information, please contact the Lumens Taiwan or Lumens UK site for details.

Service Request Form

Please complete and fax this Service Request Form with original invoice as proof of purchase to 510.252.1389. A case number and return instruction will be issued to you via email once we receive your Service Request Form.
